[Valvula] Rate limiting with postfix & valvula

That's the case: rate limiting to authenticated users. If SMTP
transaction is not authenticated, rate limited (mod-mquota) is not
applied.

It uses an in-memory database to track rate about sending users. A
database for SASL exceptions:
